WHAT
CAN WE DO FOR YOU?
- Conduct a
comprehensive assessment evaluating the client’s physical,
emotional and social well-being, family relationships, living
arrangements, finances and legal affairs.
- Establish
a personalized care plan based on our professional assessment
of your needs and capabilities.

- Conduct regular
reviews of the care plan and attend care management case meetings
to make appropriate adjustments to the client’s changing
needs.
- Continue
to monitor the client once they begin to receive new services
in the home or move into a new facility, to ensure proper care
is being provided and to prevent unnecessary crises and costly
hospitalizations.
- Report to
family members the care and support being provided.
- Act as a
liaison to families at a distance, providing assurance that their
loved one is being cared for and alerting them to any problems.
- Assist with
and file applications for benefits, entitlements, services and
community resources.
- Provide guidance
and facility profiles for selecting alternative housing arrangements
such as home care, assisted living and nursing care, supplying
information regarding deposits, waiting lists, nursing standards
and eligibility requirements.
- Review financial,
legal or medical issues and provide referrals to reputable professionals
to prevent future problems and help conserve assets.
- Handle routine
financial transactions, provide assistance with money management,
supervise bill payment and sort through mail.
- Provide conservatorship
or guardianship assistance.
- Assist with
household management including home safety evaluations, arranging
lawn maintenance, light housekeeping and home repairs.
- Assist with
routine errands such as picking up prescriptions, veterinary care
for a pet(s), grocery shopping and special occasion shopping.
- Assist with
travel planning and entertainment.
- Assist the
client with moving to an alternative living arrangement while
providing options on ensuring the client continues to keep his/her
home, if desired.
- Provide consumer
education, advocacy and support.
- Last, but
definitely not least, show the client that accepting help does
not mean you are helpless and being independent does not mean
you have to be alone...we are here for you and we will help you
achieve the peace of mind you and your family deserves.
